Summary

Group Health Insurance

Group health insurance is available to all Associates who are consistently scheduled to work 30 or more hours per week and their dependents.  Eligibility starts the first of the month after 90 days of active employment.  An optional group dental plan is also available.

Life/Accidental Death Insurance

Group life insurance equal to one times annual  base salary (subject to a maximum) is provided by CHCS for all Associates who are consistently scheduled to work 30 or more hours per week. 

Long Term Disability

Long term disability insurance equal to 50% of base monthly income (subject to a maximum) is provided by CHCS for all Associates who are consistently scheduled to work 30 or more hours per week after 90 days of continued disability.

Optional Insurance Plans

Supplemental life, cancer, disability, and vision plans are available to Associates who are consistently scheduled to work 30 or more hours per week.  Eligibility starts the first of the month after 30 days of active employment.

Health Care and Dependent Care Flexible Spending Accounts  (Section 121 and 125)

Associates who are consistently scheduled to work 24 or more hours per week are eligible to participate in these accounts.  Expenses for documented medical, dental, vision or dependent child care expenses are reimbursed from pre-tax salary reductions.  Eligibility starts the first month after 90 days of active employment.

Accrued Time

Accrued time (AT) is the Paid Time Off Plan (PTO) that provides for paid time off for vacation, holidays, and sick leave assuring maximum flexibility  for Associate’s scheduling time off from work.    Accrual begins on the first day of employment and is based on hours worked.  AT may be used upon completion of 90 days of active employment, with provisions for paid time off for holidays during the first 90 days of employment.

Bereavement Leave

CHCS recognizes the importance of family.  In the event of a death of an immediate family member, CHCS provides paid leave for up to 3 regularly scheduled work days (not to exceed 24 hours) to help make funeral arrangements and attend services.  Extended benefits are available for the death of spouse or child.

Paid Health and Wellness Benefit

CHCS provides a Health and Wellness Benefit up to $250 per year for Associates who are consistently scheduled to work 30 or more hours per week and $125 for Associates scheduled to work less than 30 hours. Eligibility starts the first of the month after 90 days of active employment.  Eligible expenses include LifeCare Wellness Center membership and nutrition consultations, vaccinations and other wellness related exams and tests provided by CHCS providers.

Associate Assistance Plan

CHCS provides up to 3 confidential counseling sessions per year for all Associates and immediate family members.  Eligibility starts immediately upon hire.

Retirement Plan

CHCS sponsors a 403(b) defined contribution pension plan and contributes  25 cents for every dollar the Associate contributes up to  4% of the Associate’s gross wage/salary.
